Is this ok as health declaration form? I hvae uploaded it for three times but was not accepted. The format is pdf and file size is 701 kb. View

@EMGS, I’ve received a mail just now which states that the health declaration form is open still. But i’ve submitted the form you provided earlier with putting tick mark on ‘yes’ box. I actually can’t understand the problem. Please intstruct me properly.

@abdullah_200 The format you sent the file in does not meet the requirements. We accept the submission of additional documents in either PDF or JPEG format only. Kindly resend the file in either of these formats.
