Clarification Regarding Health Declaration Form in EMGS Application

Dear International Student Centre,

I hope you are doing well.

I am writing regarding my EMGS application. The portal currently shows a required action stating that my Health Declaration Form is in the old format and that I need to resubmit the latest version.

However, I have already uploaded the latest Health Declaration Form. Additionally, my application status indicates that Universiti Malaya has approved my application.

Could you kindly verify whether the correct Health Declaration Form has been received? If the uploaded form is not the latest version, I would be grateful if you could let me know which version is required or advise me on the next steps.

  Kindly be informed that you may refer this link https://visa.educationmalaysia.gov.my/media/cms_upload/Lampiran_B_-_Health_Declaration_Form_For_ApplicantV2.pdf to download the latest version of health declaration form. Once fill up, kindly submit the document for us to resume the process.

    Sir are we supposed to to fill it with pen i mean print it and than write on it or we should edit the pdf . Please help
