Dear Team
My passport number is N9277314.
I was told that since I have mentioned certain illness if I wish to proceed with the health form that was submitted.
I had sent an email confirming that I would like to proceed with the health declaration form that was already submitted during the application process. But was not accepted.
Please advise on how to proceed and if there is a specific method to confirming.

  Hi Tharindi Rangoda,
  Please be informed that the applicant has declared that he/she is not free of ALL of the diseases/conditions stated in the Health Declaration Form. Therefore, kindly give your confirmation to the institute if you would like to proceed with the provided document. Alternatively, the institute may submit a new one to proceed further.
