No, previously there was something called “pre arrival medical report” in which you do what you said.
But now you just fill the health declaration form, and when you arrive to malaysia you will have to undergo a medical examination in Malaysia.

Hi Nelson,
As mentioned by @mohamed_159, you are required to submit the health declaration form only. However, you will be required to attend the compulsory post-arrival medical screening upon your arrival in Malaysia.
