I would really appreciate if you fill me up with something about INTERNATIONAL VACCINATION Certificate. Because currently I am able to arrange International Polio vaccination Certificate. Will that only work? Or are there more of vaccination certificates required?

Do I need to send the 5 pages form? Or do I have to send an international vaccination certificate?
