Hi Emgs,
Im having a problem with my visa application.
So I’m supposed to upload the latest health declaration form. As the issue suggests that i have uploaded the previous / older format.

I have downloaded the health-declaration form from the EMGS official website ( medical-screening). Both are identical.

The only difference is that i have renamed the pdf.

The contents are identical.

Please guide me regarding this matter and if there’s another form that i can download.
