Hello EMGS,
I received a 15% yellow remark stating that “the name and passport number on your passport do not match the medical form.”
However, when I check the medical form that I submitted to my university, everything is correct — my name and passport number match exactly.
I would like to understand what might have happened.
Is it possible that the university uploaded the wrong file or an incorrect version of the form?
What should I do next?

    Kindly be informed that the health declaration form submitted belong to another applicant. As such, kindly request the institution to submit the correct document in order for us to resume the process.
