1. I want to ask, “How should I write my name on the health declaration form?” I wrote my full name as it was wrote on my passport” as the applicant’s name in the health declaration form.
2. I have selected the “yes” option to declare that I am free of the indicated disease stated in the health declaration form. I wonder if this is the correct choice to declare that I am a healthy person from all these diseases listed in this form.
