I am a US citizen on a student visa looking to obtain dependent pass visas for my dependents. A requirement from EMGS and Malaysian Immigration is a “relationship letter” which apparently needs to be on an embassy letterhead and certifies my relationship to my dependents.
Unfortunately, the US embassy doesn’t issue such documents and only can notarize a self sworn affidavit with supporting documents such as a birth certificate, marriage certificate, etc.
Would a self sworn affidavit with attached documents such as a marriage or birth certificate that prove my relationship to my dependents suffice?
I have attached an example affidavit I was able to obtain so please let me know (contained within each affidavit packet I self swore to were official documents that prove my relationship to my dependents).
