    Hi @EMGS,

    My application is stuck at 15%. I have been notified about an issue related to healthcare declaration form.

    Do I need to fill up this health declaration form and submit it to you via email?

    I just need to put the tick marks on the Self column and add my signature from the DocuSign software in the document. Am I right?

      Tick mark on the box and signature then mail to your university and ask them to upload

      Hi Ehsanul,

      You are required to fill up the form and send it to us with complete details to enquiry(at)emgs.com.my. Note that this is applicable only if you have made the application by yourself.

      Otherwise, please send it to your institution if they created the application on behalf of you.
