Hello!
I’m facing a problem with my pre-VAL Medical report. On the vaccination section, I’ve had shots of evert vaccine except Yellow fever but the supporting documents of the vaccine were lost during a big move. How do I fill up the form now as I don’t have the dates for it?

  Yes this is main issues also because mostly we take the shots when we were under 1 year old and mostly the certificates were lost already. My parents confirmed i took all the vaccinations but we switched houses few times that the cert all lost. So we have to redo the vaccinations again now after 20 plus years.

  I just found records for only 2 vaccines. No records for the rest. Do I HAVE to bring the Vaccination Certificates with us or send a copy?
