I couldn’t load the image but if you mean the emgs medical check up form where they ask for vaccinations, for me the doctor refused to fill the form till I brought the official vaccination booklet where they wrote what they vaccinated me when I was born.. so I filled them up accordingly (I had two missing which aren’t a must)

Yes, the doctor said I should take my vaccination with me to Malaysia because they may need to check it during the post arrival medical check up

I didn’t submit it certificate. The report already has a part where the vaccines are listed.
But you have to take the certification with you to Malaysia.
