You just need to write the name in the format present in MRZ . Like if your name is written Jetaprakash Joshua then it should mentioned the same as in mrz field of the form you are filling.
Can you send me an example with your passport, please? It would be a great help if you can.In the MRZ surname is first then given name is there next to it. In between << symbol it there.
You can send me a snapshot of only mrz part and I can tell you .

Kindly login to your account in visa.educationmalaysia.gov.my and click on “Apply fo Student VIsa Online”. You will find a question mark on the “Applicant name as in Passport MRZ format ”.
