Good day EMGS officer,
I have been notified to submit a heath declaration form and my institution can either submit a pre- arrival medical survey report. I want to ask if the medical certificate I attached with my application is not sufficient enough. My name is Ola Olumide with passport no A12631318. My institution is UTHM.

  As per checking, we have not received the health declaration form that is requested for your application. Kindly liaise with your institution and submit the documents as soon as possible in order for us to resume the process of your application.
