As far as I understood, if it is green 35, I assume u have already submitted declaration of accommodation.
Let’s go forth:
1. Yup, u need. But not now, as u will need to present the Eval. As far as I take it, every country that have Malaysian embassy in his country (not nationality only) needs visa. But chech which type of visa do Indians need i.e. there r d/t ways to get SEVs.
2.As long as all documents are error free, Nope inshallah.
3. Well, I believe at the time of SEV application, depends if they will ask for Accommodation & flight tickets to begin process ( plus other docs).
To be on the safe side, call ur malaysian embassy, & ask them the main docs required. BTW, they will also accept offer letters, accommodation & emgs letter as add docs.

BTW, for the documents required, U can refer or ask our friends below discussion 8hrs ago from India.

You will have to first wait for your eval. Once you get the eval then you can apply for sev.
