What i know is.. if you doing home quarantine you have to submit rent contract or bills to show you are renting there. The proof is needed
Thats why I asked because if the home adress is needed I assumed a proof for that adress is needed, thanx

For your question about the vaccination certificate, it should be in English and the vaccine should be one of those recognized by WHO. Register an account through the web client of mysejahtera then upload the certificate
Wait for 3 days until it is updated.
