My Health Declaration Form was marked in reversed order. I mistook NO for YES, my bad. I noticed it when I was contacted by EMGS through email, requesting for my confirmation to proceed with the Form as it is. I submitted the corrected Form to #########, but replied that all additional documents should be submitted to #########. I immediately sent the corrected form to that email with the scanned copies of all the diagnosis tests I went through to strengthen my the corrections I effected on the new form. However, I received a second email with notification 1 from ######### about the same issue. Please I would like you to assist me on how to go about this issue. I am sure you will. Thank you.
