@Muzammill you will undergo almost same medical test that you may have taken (pre-arrival) medical here in Pakistan. The tests includes a number of blood tests to ensure you are free HIV, Hapatitis etc. Second they will conduct your vision test. Third Chest X Ray and lastly a discussion with and check up with the doctor to ensure you are sane and stable.
btw, when are you planning to fly into Malaysia? I remember you were granted VAL quite awhile ago.

Yes here is a fee. It ranges between MR 200 – 300. Usually it’s done on the university campus if you are going to a Public university. In case you are going to a private university, you will given a list of approved medical centers in Malaysia where you can go for medical.
