Yeah they do like that, and the main thing is they won’t give you any reason why they rejected it, better you try again and with good institution and submit valid documents and I hope your application will be successful

Is it pre arrival medical test or after arrival, if pre arrival then you need to do it in listed clinic from emgs that you have in your country, just go there and said about it and they will let you know all the procedure and if after arrival here, then your institution will let you know which where and when you need to do medical test

You need to contact the institution, only they can request emgs to cancel it

It’s showing rejected, that mean immigration department of malaysia rejected your application for studying here in Malaysia, you may couldn’t submitted the documents they asked or either your documents aren’t eligible for study in Malaysia
